Of course not everyone has 99 defence, and some people get hit more than others. If you have 70+ defence, simply use barrows helm and legs. They will not hit much on you. For people with less than 70 defence, there are a few options. Either use lower armour, or use prayer. When praying, a bit of defensive gear is still useful, as you won’t need to pray with full hp.
